What Is Transition Planning?

Transition planning is a process mandated by the federal law, Individuals with Disabilities Education Act (IDEA), for all students who have an Individualized Education Program (IEP) in K-12 education. The purpose of transition planning is to facilitate the student’s move from school to life after high school and post-school activities. By the specifications of IDEA transition planning must:

  • begin no later than the first IEP to be in effect when the child turns 16
  • be based on the student’s strengths, preferences, and interests
  • be individualized
  • include opportunities to develop independent & functional skills needed for work & community life

IDEA regulations (34 CFR§300) provide language in six sections that specifically refers to transition planning when it comes to a student’s IEP. These sections of IDEA cover important transition topics such as when to begin including transition goals and objectives in the student’s IEP, IEP team composition, transition services, transfer of rights at age of majority, requirements for exiting high school programs, and transition services funding.
